WALK IN CLINIC IS MOVING
When?The last day seeing patients at this location will be Sep 30.
Where?We are moving to 225 John Street South, in the Corktown Plaza, next to Remedy’s Rx Pharmacy. The clinic will be under new management. We expect the new location to open during October.
Where do I go if you’re closed?There are a number of other walk-in clinics in the area – ask staff for a list, or Google ‘walk in clinics in Hamilton’. If you have an emergency, the nearest emergency department is at St Joseph’s Hospital – two blocks South up James Street from us. There are Urgent Care centres at 690 Main St W (905.521.2100 x72000), and 2757 King St E (905.522.1155).
We encourage you to obtain a family physician if you do not already have one. For help ask our staff, the Hamilton Academy of Medicine (905.528.1611, hamiltondoctors.ca), or Health Care Connect (1.800 445.1822).
What about my medical records?The Clinic can provide you or your physician with a copy of the medical record. A fee may apply, as per Ontario Medical Association guidelines. If the clinic is closed, medical records will still be kept secure and confidential, and the Hamilton Academy of Medicine (905.528.1611) can provide information on how to access the medical record. After the new location opens records will be available from there.
What about my referrals and lab and imaging results?We will try to inform all patients about referrals before closing. Where this is not possible, we will give you the contact information to reach the specialist’s office directly. Any lab & imaging results not received before closing can be obtained from the lab or imaging office. Please follow up results with one of the clinic, urgent care, or family doctor resources above.
